ECFMG and the U.S. pathway for Caribbean MD graduates

How accreditation, WDOMS listing, and ECFMG certification relate for students aiming at U.S. residencies — without fake precision.

Page last reviewed 2026-11-02

Short answer: Graduates of many Caribbean MD programs can pursue ECFMG certification and the U.S. residency match if their school meets ECFMG's eligibility rules at the time they apply — which increasingly hinges on accreditation by a WFME-recognized agency and a correct World Directory listing. Rules change; verify on official ECFMG / Intealth pages before deciding.

What is ECFMG certification?

ECFMG — the Educational Commission for Foreign Medical Graduates — certifies IMGs (international medical graduates) for entry into U.S. graduate medical education programs (residencies and fellowships). Without it, most IMGs cannot participate in the NRMP Match. ECFMG is managed by Intealth.

This is separate from state medical licensure — which has its own requirements beyond ECFMG certification.

The pieces — keep them separate

Do not collapse these into one "accredited = can practice in the US" slogan:

  1. School accreditation — CAAM-HP, ACCM, NVAO, GMDC, IAAR, etc. See accreditation guide and the registry table.
  2. World Directory of Medical Schools (WDOMS) — listing and sponsor notes that affect exam/certification eligibility.
  3. ECFMG certification — required for most IMG pathways to U.S. residency.
  4. State and hospital requirements — clerkship approvals, state-board eligibility, and program filters that go beyond ECFMG.

What changed with the 2024 ECFMG policy?

ECFMG updated its eligibility requirements effective 2024 to tie certification eligibility more tightly to WFME-recognized accreditation. The key external sources to check:

These three sources govern your eligibility — not this registry, not a school brochure.

What to verify on a school page

On each school profile in this registry, check:

  • Current accreditation body and exact status (conditions, provisional, withdrawn)
  • Primary source links — agency list, not a blog
  • Title IV field (Yes / No / Unknown) — federal loans are a separate federal process
  • Your own confirmation of WDOMS / ECFMG sponsor notes on official directories

We intentionally leave sparse notes when we lack a primary citation. Unknown beats a wrong claim — see methodology.

Practical checklist before you enroll

  • [ ] Confirm the school's accreditor is WFME-recognized (check current WFME list)
  • [ ] Confirm ECFMG eligibility policy for your projected graduation year — not just the current year
  • [ ] Confirm clinical training geography (U.S. vs elsewhere) and any state clerkship approvals you need
  • [ ] Ask the school for a precise definition of Match statistics (who is in the denominator)
  • [ ] Budget for exams, applications, and living costs beyond tuition — see cost guide

Primary sources (start external, not here)

  1. ECFMG eligibility documentation
  2. Intealth / ECFMG official site
  3. World Directory of Medical Schools (WDOMS)
  4. WFME Recognition Status
  5. CAAM-HP assessed programs
  6. ACCM assessed programmes
  7. U.S. Department of Education foreign school lists

Big 4 often discussed for U.S.-oriented pathways: SGU, Ross, AUC, Saba — compare them on Compare. Accreditation bodies differ across that set; open each profile. SGU holds GMDC (Grenada); Ross and AUC are CAAM-HP; Saba is NVAO. Each pathway implication must be checked against current ECFMG rules.
